NORFOLK, Va. — The Virginia Department of Health in Norfolk wants residents to fill out a survey to help the department create better plans to improve the city's well-being.
The survey will ask you to identify important health issues in the community. It also asks about how your health was in the past year.
The 20-question survey is confidential and it'll take you about five minutes to complete.
